Home safety visits - before being discharged, we assess your new living arrangement to check for safety issues.

Our dedicated social services staff provides help in many ways including applying for medicaid, locating documentation, filling out medical and legal forms, etc. For those who choose to move to our assisted living facility, Legacy Place, we will help set up the home health care services that will allow you to remain as independent as possible.
